Treatment for Stage IV uterine sarcoma

Question

Dear OncoLink "Ask The Experts,"
I am recently diagnosed with stage IV uterine sarcoma. I did not respond to my first chemotherapy with Adrimyacin and Cisplatin. Are there other drugs to treat me?

Christina S. Chu, MD, Assistant Professor of the Division of Gynecologic Oncology at the University of Pennsylvania Health System, responds:

This is a difficult question. Generally, recommendations for a patient in this situation depends on the type of sarcoma --either leiomyosarcoma, Malignant Mixed Mullerian Tumor (also called carcinosarcoma), rhabdomyosarcoma, etc, as well as the locations of tumor spread. Depending on the type of tumor and the patients' general health, ifosfamide and mesna may be one option, but I would recommend discussion of this in depth with your oncologist and seek a second opinion if necessary. Radiation is sometimes an option for localized areas or for symptom control, but these decisions are all very individualized. At this point, it is reasonable to consider treatment on a Gynecologic Oncology Group clinical trial for persistent uterine sarcoma, which may be available at a local hospital.
