Disease Specific : Clinic sees survivors of a specific type of cancer or treatment., Consultative Service : One time, or additional upon request, visit to address survivorship issues, provide treatment summary, care plan and refer to subspecialties for further evaluation)., Integrated Care : Survivorship visit integrated with treating oncology team.
