Survivorship Education

Select the survivorship information for your patient's treatment binder from the list below. You can then add medication teaching sheets by clicking the "Rx Sheets" tab at the bottom. You will then click the "Cover page" tab at the bottom, where you will be able to personalize a cover sheet for your packet. The binder will open in a PDF file that you can save electronically or print.
